Everlywell Weight Care+ through Quest

How it works

Purchase through Quest, then use the enrollment link emailed by Everlywell to schedule your initial virtual consultation. Your clinician reviews your history, lab results and goals before recommending a care plan. Prescriptions depend on clinical eligibility and are not guaranteed.

Price

The initial consultation and first month cost $99. Continuing membership costs $99 per month, paid directly to Everlywell. A reminder is sent seven days before renewal. Medication and lab tests are separate expenses.

Required lab testing

This offer requires a Quest Weight Loss Journey Basic, Expanded or Premium panel purchased with enrollment or within the preceding six months. Testing must be completed before weight-loss medication can be prescribed. The required panel provides a comprehensive metabolic panel, hemoglobin A1c, thyroid-stimulating hormone and a lipid panel. Additional ongoing tests may cost extra.

Clinical care and medication

Licensed nurse practitioners and board-certified physicians provide virtual care. Follow-up visits can address progress, medication tolerance, lab testing and prescription refills. If prescribed, medication is filled at a pharmacy of your choice. Everlywell does not sell the medication. Seeing the same clinician at every visit depends on availability.

Eligibility and availability

Quest purchases are limited to ages 18 through 74 and are not permitted in Arizona, Hawaii or Puerto Rico. A clinician must assess program and medication eligibility. Purchasing the service at age 18 does not establish medication eligibility; confirm the prescribing requirements with Everlywell. Confirm service availability in your state before paying.

Cancellation

The first month, including the initial consultation and eligibility review, is nonrefundable. Contact Everlywell at least 24 hours before the next renewal to cancel. Prior months are not refunded.

Privacy and support

Program exclusions

Everlywell applies the following eligibility exclusions to this offer. Your clinician also assesses the requirements of any proposed medication.

• Under 18 years of age
• Over 74 years of age
• BMI under 25
• Currently pregnant, possibly pregnant, or actively trying to become pregnant
• Currently breastfeeding or bottle-feeding with breast milk
• Type 1 diabetes*
• Type 2 diabetes with insulin use*
• End-stage kidney disease (on or about to be on dialysis)
• End-stage liver disease (cirrhosis)
• Current or prior eating disorder
• Current suicidal thoughts and/or prior suicide attempt
• Cancer (active cancer diagnosis, under active cancer treatment, or in remission or cancer-free for less than 5 continuous years)
• History of organ transplant on anti-rejection medication
• Severe gastrointestinal condition
• History of pancreatitis
• Currently untreated/unmonitored opioid, alcohol, or other substance use disorder/dependence
• Use of blood thinning medication warfarin (Coumadin/Jantoven)
• Heart attack or stroke within the last 12 months
• Uncontrolled diabetes (HbA1c > 10%)
• Diabetic retinopathy
• Personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2 (MEN2)
